Understanding Tinnitus: What It Is and Why It Happens

Tinnitus is the experience of hearing sound when no external sound is present. People describe it as ringing, buzzing, hissing, clicking, or roaring in one or both ears. According to the National Institute on Deafness and Other Communication Disorders, about 15% of American adults report some form of tinnitus. In Iowa, this means roughly 400,000 to 500,000 residents experience tinnitus symptoms at some point in their lives.

The sound you hear with tinnitus comes from within your auditory system, not from outside sources. Your brain receives signals from the inner ear, auditory nerve, or auditory processing centers that it interprets as sound. This differs fundamentally from hearing an actual sound in your environment.

Tinnitus has many possible causes. Exposure to loud noise—whether from work, concerts, or recreational activities—ranks as one of the most common causes. Over 26 million Americans have noise-induced hearing loss, which frequently accompanies tinnitus. Other causes include age-related hearing loss, ear infections, earwax buildup, head or neck injuries, certain medications (including some blood pressure medications and antibiotics), and underlying health conditions like high blood pressure or thyroid problems.

Some people experience tinnitus that pulses in rhythm with their heartbeat. This type, called pulsatile tinnitus, may indicate a vascular condition and warrants medical evaluation. Non-pulsatile tinnitus—the most common form—has various causes and treatment approaches.

The impact of tinnitus varies widely. Some people barely notice their tinnitus, while others find it severely disrupts sleep, concentration, or emotional wellbeing. The relationship between the tinnitus sound and how distressing it is depends on factors like the loudness, pitch, how long you've had it, and your emotional response to it. Understanding your specific situation is the first step toward exploring treatment options.

Practical Takeaway: Keep a brief log of when your tinnitus started, what it sounds like, which ear(s) it affects, and what makes it better or worse. This information helps healthcare providers understand your situation and suggest appropriate treatment paths.

Medical Evaluation and Diagnosis in Iowa

If you experience tinnitus, scheduling an evaluation with a healthcare provider is an important first step. In Iowa, you can start with your primary care doctor, an ear, nose, and throat specialist (otolaryngologist), or an audiologist. Each professional brings different expertise to understanding your tinnitus.

Your primary care physician can perform an initial assessment, review your medical history, check your medications, and examine your ears for obvious problems like earwax impaction or infection. They may refer you to a specialist if your situation requires more detailed evaluation. Iowa has otolaryngologists located in major cities including Des Moines, Cedar Rapids, Iowa City, and Sioux City, as well as in smaller communities.

Audiologists are hearing healthcare professionals who specialize in evaluating hearing and balance. They can perform comprehensive hearing tests to determine whether hearing loss accompanies your tinnitus. Many audiologists in Iowa work in hospitals, audiology clinics, or hearing aid dispensing offices. A hearing test, called an audiogram, measures how well you hear at different frequencies and volumes. This information is valuable because hearing loss and tinnitus often occur together, and treating one may affect the other.

During your evaluation, healthcare providers will ask detailed questions: When did the tinnitus start? Was it sudden or gradual? Do you have hearing loss? Have you had recent head injuries? Are you taking new medications? Do you work in a noisy environment? What makes the tinnitus better or worse? They may perform otoscopy (looking in your ear with a lighted instrument) and order hearing tests or imaging studies depending on their findings.

Some tinnitus cases have identifiable, treatable causes. Earwax impaction, for example, causes tinnitus and is easily treated by a healthcare provider. Certain medications cause tinnitus as a side effect; your doctor may adjust your dosage or switch to an alternative. Infections or ear problems can sometimes be resolved with treatment. However, for many people, particularly those with age-related or noise-induced hearing loss, the underlying cause cannot be reversed. In these cases, treatment focuses on managing the tinnitus experience itself.

Practical Takeaway: Contact your primary care doctor or search the Iowa Medical Society website to find local otolaryngologists and audiologists. Schedule an evaluation to identify whether your tinnitus has a treatable cause and to establish a baseline understanding of your hearing.

Sound-Based Treatment Approaches

Sound-based treatments are among the most researched and widely used approaches for tinnitus management. These treatments work on the principle that introducing external sounds can reduce the notice-ability and distress associated with tinnitus.

Sound Masking and White Noise: Sound masking uses external sounds to cover or mask tinnitus, making it less noticeable. White noise machines, fans, nature sounds, or specially designed tinnitus masking apps can provide this benefit. Many people find that using a sound machine while sleeping helps them rest better, since tinnitus often seems louder in quiet environments. These tools range from inexpensive (apps that cost nothing or a few dollars) to specialized tinnitus masking devices.

Hearing Aids: For people with both hearing loss and tinnitus, hearing aids often reduce tinnitus perception. Hearing aids amplify environmental sounds, making external sounds more prominent relative to tinnitus. Additionally, many modern hearing aids include tinnitus management features such as built-in sound generators or apps that deliver customized sounds. Iowa audiologists can fit and program hearing aids tailored to your hearing loss and tinnitus characteristics.

Combination Devices: Some devices combine hearing aid amplification with sound generation in a single instrument. These are particularly useful if you have hearing loss along with tinnitus.

Notched Music Therapy: This emerging approach involves listening to music with a frequency notch—a gap—at the frequency of your tinnitus. The theory suggests this may reduce neural activity related to tinnitus. While research is ongoing, some people report benefit from this method.

Tinnitus Retraining Therapy (TRT): TRT combines sound therapy with counseling to help your brain habituate to tinnitus—essentially teaching your nervous system to filter out the tinnitus sound as unimportant, similar to how you stop noticing background traffic noise. Some Iowa audiologists and hearing specialists offer counseling components that align with TRT principles.

Practical Takeaway: Experiment with different sounds—white noise, nature sounds, or music—to discover what works best for your tinnitus. Many free or low-cost options exist through apps or simple devices before investing in specialized equipment.

Behavioral and Psychological Treatment Methods

How you think about and react to tinnitus significantly affects how much it bothers you. Behavioral and psychological approaches address the distress and disruption tinnitus causes, even if they don't change the sound itself. These methods help rewire your brain's response to tinnitus.

Cognitive Behavioral Therapy (CBT): CBT is one of the most studied psychological treatments for tinnitus. Research shows it can reduce the distress and disability associated with tinnitus. In CBT, a therapist helps you identify unhelpful thought patterns about tinnitus—such as "this will never go away" or "this means something is seriously wrong"—and develop more balanced, realistic perspectives. You also learn practical coping strategies for managing tinnitus-related stress and anxiety. Many therapists in Iowa offer CBT, though you may need to specifically seek out those with experience treating tinnitus or hearing-related conditions.

Acceptance and Commitment Therapy (ACT): ACT focuses on accepting the presence of tinnitus while committing to living a full, meaningful life. Rather than fighting against the tinnitus or trying to eliminate the sound, you learn to change your relationship with it. This approach reduces the struggle against tinnitus, which paradoxically often reduces its perceived impact.

Mindfulness and Meditation: Mindfulness-based approaches teach you to observe tinnitus without judgment or emotional reaction. Regular practice can reduce
